Jungle Gym Preschool

Important Information: 

Children must be signed in daily. Changes in persons allowed to pick up your child must be made in writing and given to the staff in person. If we do not know the person picking up your child they will be asked for photo I.D.. The person's information will then be checked agaist your enrollment information. These policies are in place for the safety of all the children in our care.

The Pacific Clinic Release Waiver: 

I hereby waive and release any and all rights for myself, my heirs, executors and administrators this enrollee may have against The Pacific Clinic or it's representative, agents and successors for any and all injuries the participant may suffer in connection with his/her participation in any and all preschool programs at The Pacific Clinic. 

 

Medical Care Waiver: 

I understand that in case of an emergency and we are unable to be contacted, I give permission to The Pacific Clinic to authoize any emergency action necessary to ensure the safety of our child. This does not in any way hold The Pacific Clinic finacially responsible or otherwise liable for any medical or emergency care given. I permit my child to participate in all activities at The Pacific Clinic Jungle Gym Preschool and to be transported to off-site field trips. 

 

Photograph/Image Release: 

The Pacific Clinic occasionally uses photographs/images of clients for the purpose(s) of newsletter aricles, in-house promotions, marketing and website postings. The function of this release is to establish approval by the subject for these specific uses. I agree to allow The Pacific Clinic to use images for in-house newsletters, marketing, brochures, website, etc. Jungle Gym Preschool will be taking pictures and video to email parents occasionally. We would like to share some of the fun activities with parents by email. I agree to allow The Pacific Clinic Jungle Gym Preschool to release pictures and videos of my child for this purpose.
